A home health professional assists a TAP clientThe Alternatives Program (TAP) provides in-home services to medically frail adults who qualify and are at risk for premature placement in a nursing home.

The program’s goal is to help keep frail adults in their homes by providing and coordinating a variety of services. The customer’s needs are first assessed by a social worker and a nurse who develop a personal care plan to address these needs.

The program then contracts with private organizations, including home health agencies, to provide a wide variety of services such as home health aides, homemaking, emergency response systems and other services as needed. The program also provides Service Coordinators to assist seniors who live in residential sites for the elderly.
